Financial Arrangements
We will discuss your financial
obligations and responsibilities in a private and confidential manner.
If you have an insurance plan, we will submit our recommendations to your
insurance provider. If you are paying on a fee-for-service basis, we
will work with you on how you can best receive the care you deserve.
Option A: Payment in advance
of treatment plans over $1000 will receive a 5% discount
Option B: Pay as You Go: You
may choose to pay your obligation for each visit, at the visit.
Option C: Monthly Budget Plan

monthly payments, a credit card number may be left on account.

The monthly amount and day of billing
will be conveniently worked out for you by Marcia

Option D: Combining Insurance
Benefits and Out of Pocket

Insurance plans cover many of the
procedures we recommend on a partial basis. Some procedures,
depending on your condition, can be completely covered. However,
we recommend treatment plans based on your optimum care needs.
Insurance providers can base much of your coverage on your premium
payments. State-of-the-art esthetic and restorative dentistry can
conflict with an insurance plan's view of "necessary" dentistry.
All dentistry should be looked at as elective dentistry because it all
comes down to whether you decide it is for you.

It is our pleasure to assist you in
maximizing your insurance benefit. Please keep us updated with our
current insurance information.

Our office does not accept
insurance as payment in full. As a courtesy to our patients,
our office will submit your dental insurance claims. You will be
expected to pay an estimated co-payment at the time of your visit.
Please keep in mind, any estimate we provide for you, is only an
estimate. We will bill you for any difference.

We can help you determine the
approximate portion of fees your insurance will cover. If
necessary, we will file a pre-estimate form to determine the amount of
benefits you will receive for treatment. As a courtesy, in
addition to filing the claim, if you prefer we will initially ask you
only for your estimated co-payment. Please understand that this is
only an estimate, and is based upon the information available to us.
The financial obligation for dental
treatment is between you and our office, the insurance company is
responsible to you, and not to our office. We will assist you in any
way that we can. Once your carrier has paid the claim, any difference
will be due at that time.

orders, and debit cards.
